Patients

The following information is primarily for patients seeing Dr. Rathbone at the Neurology Clinic for the first time. We want you to get the most out of your visit to the clinic. Please see the list below to learn how you can help make your visit more progressive.

1. When you think of questions before your appointment, write them down and bring them with you to your appointment. Bring with you, your medicines in their original containers. Bring a list of all doctors who are treating you. I will send a copy of my consultation letter to each, where appropriate. Fill out the health questionaire and bring it with you. (click here to download and complete the questionaire)You must bring your Ontario Health Insurance Card with you so we can check your Ontario Health Insurance number.
2. Bring a list of previous testing relevant to the appointment.

To contact the Doctor, the best way is via e-mail, regular mail or fax as there is not always a person in the office to take your call.

PLEASE NOTE: Secretaries are not able to discuss lab results over the phone, nor can patients speak to the doctor regarding their medical condition by phone. If this were to happen, there would be a fee charged for this service.
